Concussion Code of Conduct - Once per school year, both parent and student must submit the electronic forms below...

Form 974 - Concussion Code of Conduct - Student 

Form 975 - Concussion Code of Conduct - Parents/Guardians

If a head injury happens outside of school: Parents/guardians must complete the OCDSB 908 Concussion Medical Assessment Form which confirms whether or not the student has a diagnosed concussion.  Please return it to the main office as soon as possible by emailing the completed form to [email protected].

If a concussion was diagnosed, Parents/Guardians must complete the OCDSB 970 School Concussion Management Form at each stage of the student’s return to learning.  Parents can email the student’s guidance counsellor or VP as the student progresses through each stage. Once they’ve completed all stages, please return the completed form to the main office.

Finally, a medical doctor/nurse practitioner must complete the OCDSB 971 Medical Concussion Clearance Form once students have completed Stage 4a/4b.  Medical clearance is required before students may progress to Stage 5.  Once complete, please return this form to the main office.
